$160,000 – $180,000 annually, plus potential for a significant retention bonus Job Summary: We are seeking a bilingual (Spanish-speaking) Nurse Practitioner (NP) to join our growing team focused on delivering high-quality Medicare Annual Wellness Assessments for geriatric patients in the San Diego North County area. This hybrid position includes both clinic-based and in-home patient assessments, offering consistent weekday hours in a lower-stress environment, with no prescribing responsibilities. Job Responsibilities: Conduct detailed patient interviews and gather comprehensive health histories (medical, surgical, social, and family) Perform Medicare Annual Wellness Exams and health screenings in both clinic and home settings Administer preventive health screenings such as spirometry, circulation testing, diabetic retinal exams, and nerve testing Accurately document findings using an EMR system (eClinicalWorks) Identify and code patient diagnoses, supported by clinical evidence, and create individualized care plans Apply knowledge of HEDIS/STAR measures and Medicare risk adjustment in patient evaluations Educate patients on preventive care topics including fall prevention, screenings, and vaccinations Collaborate with physicians and care teams to ensure continuity of care Travel locally to assigned patient homes and partner clinics as needed (approx. 75% clinic / 25% home visits) Qualifications: Active Nurse Practitioner license in CA Fluent in Spanish – bilingual proficiency is required Experience with adult or geriatric populations strongly preferred Background in family medicine, internal medicine, rehab, orthopedics, pain management, or long-term care is a plus Proficient in using EMR systems (eClinicalWorks or Epic preferred) Tech-savvy and comfortable with electronic documentation Strong communication and interpersonal skills Valid driver’s license and reliable transportation required About the Company: We are a mission-driven healthcare organization focused exclusively on preventive care for Medicare patients.
